Insurance Card
You may be asked to update your personal information or complete an updated HIPAA form, please allow extra time for check-in.
If your child is scheduled for an ADHD Consult or follow up appointment, please make sure any and all Vanderbilt scales for parent and teachers are completed for doctor review.

If your child is having a life-threatening emergency, call 911. For a poison emergency in the U.S. call 1-800-222-1222.
